Required Documents

To enroll your child at Gilgandra Preschool, some documents are required prior to a position being made available.  We can make a copy of documents where required or legible documents can be emailed to the Preschool to be attached to your enrolment.

 

Documents required include:

  • Up to date Immunisation details.  This must be from myGov or what is provided from Medicare.
  • Copy of Medicare Card
  • Birth Certificate (this can be applied for and provided at a later date where required to do so).

 

Additional information where relevant

  • Copy of Health Care/Pension card where the child is listed as a dependent (cannot be the child's own card).  Providing a copy of this where relevant assists in confirming enrolment fees.
  • Court or Parenting orders.
  • Medical action plans (e.g. Asthma, Anaphylaxis).
  • Information relating to a medical condition to best assist us in providing care.
  • Information relating to a diagnosis to best assist us in providing care.
  • Any other relevant reports.
